NZSCIR – Registration of Interest
We want to reach out to the NZ SCI community to ask them questions about how their SCI impacts on their lives. This helps us understand more about people with SCI and how they are being supported. Please feel free to tell others you know who may be interested in joining the NZSCIR.
We are currently collecting the names of people who had their SCI prior to 1st August 2016.
This online form uses the NZ Spinal Trust Portal to send information directly to NZSCIR Coordinators. In completing the information below, you are consenting to us accessing your medical records to determine your eligibility to participate in the NZSCIR and collect basic information about you and your SCI (known as a minimal data set). It will also allow us to be in touch with you to discuss your participation.
